When infection around the root tip persists after a root canal and retreatment is not an option, endodontists can perform microsurgery known as an apicoectomy.
In an apicoectomy, the endodontist makes a small opening in the gum, removes the inflamed tissue and the very end of the root, and seals the root tip. Performed under a surgical microscope, it is a precise, minimally invasive way to save a tooth that would otherwise be lost. It is typically considered only after non-surgical options have been exhausted. Recovery is usually quick, with most patients managing any discomfort with over-the-counter medication.
Surgical treatment of one root tip, common for front teeth.
Surgery on molars or teeth with multiple roots.
Pre-surgical evaluation with 3D CBCT scan.
Endodontists are root canal and dental-pain specialists. After dental school they complete two or more years of accredited residency focused on saving teeth. The defining credentials are AAE membership and board certification — Diplomate of the American Board of Endodontics.
